After making your first appointment:
Below and to your right -- find either the individual or couples form, + the Informed Consent/policies form.
1. Download the two forms (individual OR couples + Informed consent)
2. It is ESSENTIAL that you read, fill out where necessary, and sign the INFORMED CONSENT ...
counseling cannot begin without your signature on this form!!
2. Fill the forms out, (each partner fills out one couples form), bring them to your session.
(It's easy, and enables us to spend the entire first session discussing your issues - NOT filling out forms!)

Directions
From I-430 ... Take Rodney Parham exit off 430, go west approximately 1/4 mile to Shackleford Road. Turn left. Pass the Cadillac and BMW dealerships on your right. Turn right at the fire station onto Shackleford Plaza. Turn right into the first parking lot on your right, at 4 Shackleford Plaza.
